Individual and Family Health

Individuals and families experiencing barriers to good health such as affordable health care, mental health support, health insurance, dental care, nutritious food, safe and affordable housing, education, can enroll in the family health program.


Services Include:

1:1 Meeting with the Family Health Coordinator and/or Community Health Worker
  • Referrals to community services
  • Advocacy
  • Assistance with accessing care

* This program is open to Myers-Wilkins families and those living in the surrounding neighborhood. 
Community Health

Community Health means having a safe, supportive place to live with opportunities to play, learn, and grow. The Community Health Team engages people of all ages in learning more about lifelong health and creating opportunities for healthy activities in our neighborhood. 

Among these are:
  • Healthy Cooking
  • Zumba & Yoga classes
  • Community Health Resource Fair
  • Leadership Development
  • Opportunities to have a voice in community decision-making
Community Health is an ongoing process that builds upon the needs and interests of families and neighbors.
